How do I report a Workers Compensation Incident?

Workers' Compensation Incidents?
All Incidents that involve a work-related injury or illness to any Tishman Speyer employee regardless of responsibility, fault, or employee error is a workers' comp claim.

Steps to Take:

  1. In an event of an emergency, such as trauma, life-threatening injury, broken bone, etc., make immediate arrangements for emergency treatment and ambulance transportation.
  2. Injuries should be reported to immediate supervisor who should complete a Tishman Speyer
    Incident Report. The supervisor should report the incident immediately or at minimum within 24-48 hours.
  3. Once the form is complete, please email the form to: IncidentReportWC@TishmanSpeyer.com - The incident report will be sent out to Julie Lydon of Human Capital Management & Adriana Galimi of WTW.
  4. Please also email the form to: Esisclaims@tnwinc.com - When reporting via email to ESIS, a specific template must be followed:
  •  Subject Line: Tishman Speyer Properties, L.P. – new (fill in blank noting state) WC claim
    for (claimant name) – VDN: 2226239
  • Text of e-mail:
    • EE Name:
    • State of loss / jurisdiction:
    • Date of Loss:
    • Building Location:
    • Policy Number: (WC reference card found on portal)
    • EE occupation: Engineer / Cleaner, etc.
    • Location / Manager / client contact: (this is the person reporting the claim)
    • Claim type: record/notice only, medical only (no lost time- treated at medical facility and back to work) or lost time)

5.  Provide TS employee with the WC Reference Card before they seek medical treatment.

Note: Even if all Information is not known, report the claim promptly with available information. You can submit additional info later.


Important:

  • Preserve evidence that may be applicable to the incident. Photos should be taken at the scene immediately after the occurrence to show the conditions at the side of the incident. Retain all relevant video footage.
  • Obtain eyewitness names, addresses and phone numbers.
  • It's important to complete all state forms in a timely manner. Failure to do so may result in
    penalties/fines levied against Tishman Speyer.
